A Medicines Administration Record — usually called a MAR chart — is an official log of the medicines a person takes at home. Care workers and nurses use it to record what was given, when, and how. It helps everyone involved in someone's care work from the same clear record.
MAR charts are for people who receive visiting care at home and need support with their medicines. In Wales, an NHS scheme provides these charts free of charge through community pharmacies, so care is safe and consistent from one visit to the next.
