Discharge Medicines Review

Coming out of hospital can leave you with a lot to take in. Your medicines may have been changed, stopped or started, and it is not always clear why. The Discharge Medicines Review is a free NHS Wales service at Vale of Neath Pharmacy that gives you time with our pharmacist to go through your medicines after a stay in hospital.

What happens at the review

You will have a private chat with the pharmacist. Together you will look at the medicines you were taking before you went into hospital and the ones you have been given since.

• We check what has changed, and why.
• We talk through how and when to take each medicine.
• We go over any side effects you have noticed.
• We answer any questions you or your family have.

If something does not look right, or you need more support, we can speak to your GP or another member of your care team.

Who it is for

The service is for people who have recently been discharged from hospital and whose medicines have been reviewed or changed. If you are not sure whether it applies to you, just ask — our pharmacist will be happy to talk it through.

Book online, or simply pop into the pharmacy and ask to speak with the pharmacist.
